Common Issues for Submitting POA Online

Please take note of the steps or possible issues in the POA Online form:

  1. Donor(s) must have mental capacity.  The fields of Donor 1  Mental Capacity and Donor 2 Mental Capacity (if applicable)  must be “Yes” in order for the “Submit” button to appear.
  2. After completion of the form, please click Next button or Save and Continue Later button (as the case may be).
  3. If you click Save and Continue Later button, you will save the form and an email with a unique link will be sent to you to complete your form at a later time.
  4. If you click Next button, you will be brought to a Summary page, and at the bottom, you will see the buttons as shown below. To continue with your submission, please click Review & Submit button to go to a final Review page. To go back to the previous page to edit your particulars, plese click Previous button.
  5. If you click Review & Submit button, you will be brought to your submission page, please click Submit button to submit your application.
  6. Once you have successfuly submitted the form, you should receive an email from on your submission and with our further instructions.
